Brand GLP-1 prices collapsed. Most comparison sites still haven't noticed.
For three years the argument for compounded GLP-1 was simple arithmetic: roughly $1,000 a month for the brand against $200 for a compound. Lilly and Novo have now closed most of that gap themselves — and the comparison industry has not updated.
Zepbound fell to $299. Wegovy to $349. The oral tablet to $149. The price gap that justified compounded GLP-1 has largely closed, and almost nobody is saying so.
What actually changed
Between November 2025 and mid-2026 the manufacturers repriced. Novo Nordisk cut Wegovy self-pay from $499 to $349. Eli Lilly's LillyDirect programme put brand Zepbound at $299 for the starting dose and $449 at maintenance. Then both launched oral products at $149 — Lilly's Foundayo (orforglipron) and Novo's Wegovy tablet.
Set against retail list prices of roughly $1,086 for Zepbound and $1,349 for Wegovy, those are reductions of 60 to 90 per cent. And set against the compounded market, they are something more awkward: competitive.

The gap that is left
Here is the comparison the affiliate industry does not run, because running it costs them money.
The two brand lines are the benchmark. Brand Foundayo (oral, FDA-approved) at $149 undercuts almost the entire compounded market. Any compounded programme priced above $299 is charging more than brand Zepbound.

MEDVi's reaches $499 at higher doses. Henry Meds' oral tirzepatide is $349. Those patients are paying a premium for a product the FDA has not reviewed, in a multi-dose vial they must measure themselves, resting on a legal basis that narrowed sharply in 2025.
The compounded case has not disappeared. It has become conditional. It holds where a programme is priced meaningfully below the brand floor — NexLife's $147 microdose and $186 standard, Oak's $133 semaglutide, Found's $169 prepaid. It does not hold at $349 or $399, and no amount of marketing changes that arithmetic.
The part nobody saw coming: the cheapest approved option is a pill
The genuinely surprising development is that the cheapest FDA-approved GLP-1 for weight loss is now oral, and it undercuts most of the compounded market.
Foundayo starts at $149/month at the 0.8mg dose through LillyDirect. The oral Wegovy tablet is $149 at 1.5mg and 4mg through NovoCare. Both are FDA-approved, quality-verified before marketing, and supplied by the manufacturer.
The catch is escalation. Foundayo rises to $199, then $299, then $349 as you titrate, with its own 45-day refill condition at the top doses. So it is not universally cheapest. But for a starting patient, or someone maintaining on a lower dose, an approved oral medication at $149 is now among the cheapest legitimate options in the entire category — and it is being ignored by comparison pages built to sell compounded subscriptions.
Why the comparison industry has not updated
It is worth being blunt about the incentive. A comparison site earns a commission when a reader signs up for a compounded telehealth subscription. It earns nothing when a reader buys Zepbound directly from Lilly.
That does not make every such site dishonest. It does mean that the single most useful piece of advice in this category — check the manufacturer's own price first — is advice no affiliate site is paid to give. We are an affiliate-funded publication too, which is disclosed in our footer, and we are giving it anyway, because a comparison that never tells you to buy elsewhere is not a comparison.
What to actually do
- Check your insurance first. With commercial coverage and a manufacturer savings card, brand Zepbound or Wegovy can cost roughly $25/month. Nothing else competes.
- Then check LillyDirect and NovoCare directly. $149 for an oral, $299-$449 for brand Zepbound.
- Then, and only then, compare compounded programmes — and only consider ones priced clearly below the $299 brand floor.
- If you are currently paying $350-$400 for a compound, re-price today. You are paying at or above brand for a product the FDA has not reviewed.

Prices decay quickly. This is the fastest-moving data we publish. Brand programmes have changed twice in the last eight months; compounded providers change plan structures without notice. Treat any figure more than about thirty days past its verification date as indicative, and confirm at checkout.
Competitor pricing is reported, not captured by us. We hold dated captures for brand pricing and for NexLife. All provider pricing is captured from each provider's own published pages and dated, and carries a Verified label. Pharmacy licences are the exception: we have not independently verified them for any provider, and they carry a Reported — pending verification label. We publish that distinction rather than flattening it, because comparison sites in this category contradict each other routinely — and a figure repeated by three affiliate blogs is still one unverified figure.
We have not audited pharmacy licences. Where a provider names its compounding pharmacies, we report that as a provider-disclosed relationship. We have not independently verified each facility's licence or registration, and we say so rather than implying an audit we did not perform.
